How does the study medication compare with my current treatment?
Once I am in the study, do I continue to see my usual physician? Should they be aware of my participation in the study?
How might this participation affect my daily life?
Will hospitalization be required?
Who will pay for the treatment?
Will I be reimbursed for other expenses?
Who will be in charge of my care during the study?
What happens if I change my mind and no longer want to participate?
What happens if I don’t get into the study, but still want access to the medication? Is that possible?
Consider Bringing to the Study Visit:
A friend or family member for support or as “another pair of ears”
Note paper and pen to take notes

A member of the study team will meet with you and ask you questions to see if you meet the study enrollment criteria:
About your overall health, and your medical history
About your condition, and what limitations it may place on your activities of daily living
It is important to mention any allergies or any other conditions you may have
The study team will tell you about the study, and what you will need to do to participate:
Physical examinations, blood tests, vaccinations
The benefit and risks of participating in the study
